Who is eligible?

- An individual who makes a positive difference in our community.
- An individual who would benefit from a confidence boosting new smile.
Help us select a worthy candidate by nominating someone whose life would benefit from this confidence building makeover.

*Three finalists will be invited to our office to meet our team and doctors for an oral assessment. Candidates must have stable physical and dental health to undergo procedures for the smile makeover. Our smile makeover gift will include an oral assessment (with digital radiographs and photos) with the dentist, an initial teeth cleaning, and up to 10 units of porcelain veneer restorations of the upper arch. Periodontal, endodontic (root canal), and or surgical dental treatment are not part of the smile makeover giveaway and will be the patient’s financial responsibility.
The selected nominee must agree to provide a written and recorded personal testimonial and use of his/her photos and videos taken during the smile makeover journey.
